Indications
Detrol Tablets are indicated for the treatment of overactive bladder with symptoms of urge urinary incontinence, urgency, and frequency.
Dosage and Administration
Detrol is available for oral administration as 1 or 2 mg tablets.
Normal dosage: The initial recommended dose of Detrol Tablets is 2 mg twice daily. The dose may be lowered to 1 mg twice daily based on individual response and tolerability. For patients with significantly reduced hepatic or renal function or who are currently taking drugs that are potent inhibitors of CYP3A4, the recommended dose of Detrol is 1 mg twice daily.
Overdose: Overdose symptoms may include dry mouth, blurred vision, and fast heartbeat.
Drug Description
Detrol Tablets contain tolterodine tartrate. The active moiety, tolterodine, is a muscarinic receptor antagonist. The chemical name of tolterodine tartrate is (R)-2-[3-[bis(1-methylethyl)-amino]1-phenylpropyl]-4-methylphenol [R-(R*,R*)]-2,3 dihydroxybutanedioate (1:1) (salt). The empirical formula of tolterodine tartrate is C26H37NO7, and its molecular weight is 475.6. The structural formula of tolterodine tartrate is represented below:
Tolterodine tartrate is a white, crystalline powder. The pKa value is 9.87 and the solubility in water is 12 mg/mL. It is soluble in methanol, slightly soluble in ethanol, and practically insoluble in toluene. The partition coefficient (Log D) between n-octanol and water is 1.83 at pH 7.3.
Side Effects
Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty in breathing; swelling of your face, lips, tongue, or throat.
Stop using tolterodine and call your doctor at once if you have any of these serious side effects:
- chest pain, fast or uneven heart rate
- feeling short of breath, even with mild exertion
- swelling, rapid weight gain
- confusion, hallucinations
- urinating less than usual or not at all; or
- painful or difficult urination
Less serious side effects may include:
- dry mouth, dry eyes
- blurred vision
- dizziness, drowsiness
- constipation or diarrhea
- stomach pain or upset
- joint pain; or
- headache
This is not a complete list of side effects and others may occur. Tell your doctor about any unusual or bothersome side effect.
Precautions
- Tolterodine can cause blurred vision or side effects that may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be awake, alert, and able to see clearly.
- Avoid drinking alcohol. It can increase some of the side effects of tolterodine.
- Do not take this medication if you are allergic to tolterodine or if you have:
- a blockage of the urinary tract (difficulty urinating)
- a blockage in your stomach or intestines; or
- untreated or uncontrolled glaucoma
- Before taking tolterodine, tell your doctor if you are allergic to any drugs, or if you have:
- liver disease
- kidney disease
- glaucoma; or
- a personal or family history of "Long QT syndrome."
- FDA pregnancy category C. This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment.
- It is not known whether tolterodine passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.
Drug Interactions
Before taking Detrol, tell your doctor if you are using any of the following drugs:
- arsenic trioxide (Trisenox);
- bepridil (Vascor),
- cisapride (Propulsid);
- chloroquine (Arelan) or halofantrine (Halfan);
- cyclosporine (Gengraf, Neoral, Sandimmune);
- droperidol (Inapsine);
- narcotic medication such as levomethadyl (Orlaam); or methadone (Dolophine, Methadose);
- pentamidine (NebuPent, Pentam);
- vinblastine (Velban);
- antibiotics such as azithromycin (Zithromax), clarithromycin (Biaxin), dirithromycin (Dynabac), erythromycin (E-Mycin, E.E.S., Erythrocin, Ery-Tab), pentamidine (NebuPent, Pentam), sparfloxacin (Zagam), telithromycin (Ketek);
- medicines to treat psychiatric disorder, such as chlorpromazine (Thorazine), haloperidol (Haldol), mesoridazine (Serentil) pimozide (Orap), or thioridazine (Mellaril); or
- heart rhythm medicine such as amiodarone (Cordarone, Pacerone), dofetilide (Tikosyn), disopyramide (Norpace), procainamide (Procan, Pronestyl), quinidine (Cardioquin, Quinaglute), or sotalol (Betapace).
This list is not complete and there may be other drugs that can interact with Detrol. Tell your doctor about all the prescriptions and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.
